the Otherheart is, in short, the physical manifestation of False Love and the undying, unyielding URGE to protect the ones you love, twisted beyond recognition, controlled by a lost soul. the spell used to create the Otherheart, albeit by accident, was actually an attempt to use said lost soul, Cosmo, to brighten the hopes and dreams of love for the people for the rest of the year, but failed due to Cosmo's grief, turning the spell into a monster, and Cosmo into a Goddess, who goes around taking others and placing them in her 'heaven' where nobody can hurt them, not knowing who she's hurting in the process, using those she's 'saved', the Contaminated, to do her dirty work for her.
Contaminated: Cosmo(duh) all of Team Sonic except Cream, Shadow, and Knuckles etc
