Date: June 12, 2004. Southwest flight 328 was a domestic passenger flight flying from San Francisco International Airport to Harry Reid International Airport. The pilot applied maximum thrust to both engines and set flaps for takeoff. The plane was gaining speed, but it was unable to gain lift. The copilot assumed it was a pitch error, and pitched the plane way too far back. This caused the tail section to separate from the rest of the plane. The nose gear then collapsed due to intense speed and the center of gravity being shifted. The nose gear door would then cause a lot of friction against the runway, causing that section of the plane to flip upside down and violently slam into the end of the runway.
